AFL is a service that allows you to invite a person with intellectual and developmental disabilities to move into your home and become a part of your household. The individuals in this program have an opportunity to become more independent with social skills, job skills, education and day to day living. The service can be provided by a family or individual in a home setting.
I/DD Evaluation for residential and day placement
(Innovations Waiver) Support the person served to live successfully in their own home and to be an active member of their community by increasing or maintaining the person’s life skills or to provide the supervision needed.
(Innovations Wavier) Provide individualized day activities that support the individual’s definition of a meaningful day in an integrated setting, with persons who are not disabled.
(Innovations Wavier) Provide for intervention and stabilization for the individual experiencing a crisis and the need for a higher level of care.
(Innovations Wavier) Support day activities in a facility-based setting separate from residential settings.
For Children 0-3 years of age.
System to provide safety net for persons served.
I/DD Services funded by your local Managed Care Organization (MCO) with state dollars.
Designed to equip adults with necessary life skills.
Alternative Family Living (AFL) & Group Home Services (call local office for more information).
(Innovations Waiver) Support successful living in a group home or alternative family living setting that meets the home and community based characteristics.
(Innovations Waiver) Provide periodic or scheduled support to the primary caregivers.
Service to assist people with disabilities find employment and for businesses to reach a valuable and largely untapped workforce.
(Innovations Waiver) Support successful community living in an unlicensed residence owner/leased by the individual (ages 18 or over) to increase or maintain life skills.
Community-Based Assessments, Supported Employment, and Work Adjustment Services.
